Join MHC Juniors - Registration

Part 1: Eligibility

You can only register for Milnerton Hockey Club if:

You are up to date with your MHC Fees
You are in good standing with both the WPHU and your previous club
Have received a clearance letter from your previous club (if transferring)

There is NO need for you to register and pay SAHA. The club will be handling all payments on your behalf. Therefore all affiliation fees will be included in your fees to MHC.

Fees include: SAHA registration, WP Hockey registration, and Milnerton Hockey Club membership.

Additional Payment Options:

If you would like to split the payments, please note the following terms:

• 50% of your total fee due upon registration
• Remaining 50% split into 3 equal monthly payments

Part 5: Consent & Declaration

By signing up to join Milnerton Hockey Club trials, training sessions, social events or clinics I hereby acknowledge the risks involved in my child's participation and therefore indemnify Milnerton Hockey Club, the venue, coaching, staff, participants, etc. from any claims related to injury, loss or breakages sustained as a result of directly or indirectly attending the Milnerton Hockey Club training sessions, social events or clinics.

I agree - By submitting this form, I, the parent/legal guardian of the registered child, acknowledge and agree to the following:

• Consent for Registration: I hereby give permission for my child to be registered for the upcoming hockey season and to participate in all related activities.
• Financial Responsibility: I understand that by completing this registration, I am committing to the necessary fees associated with the season and agree to make payment as required by the club.
• Data Protection & Privacy: I acknowledge that all personal information provided in this form will be stored securely and handled in accordance with the Protection of Personal Information Act (POPIA). This information will only be used for club-related purposes and will not be shared or used beyond the scope of the Milnerton hockey club.

Photography & Social Media Consent *

I consent to my child being photographed or recorded during club activities. These images/videos may be used for club communications, social media, or promotional materials. I understand that names will not be shared alongside any images.
